Transitional Rehab

Our Transitional Rehab program is for patients who have had a recent life altering event within the last 18 months with a significant change in function who would benefit from a burst of physical and occupational therapy. The program will have two spots at our Mcwane PT & OT location with a goal to open a PT & OT slot at our Lakeshore location in the future. Patients will receive PT and OT 2 to 3 days a week for 2 hours (1 hour of each discipline) for up to 4 weeks with a re-eval at the end of each week to assess progress. At the end of the program the goal is to transition to a community therapist if still appropriate for skilled therapy. Outpatient therapy may continue at COA if the family desires with the understanding that schedule and/or therapists may change. The goal of this program is to provide patients with appropriate therapy and frequencies within a timely manner as well as a smooth transition from inpatient to community/ outpatient therapies. 
 

Inclusion Criteria:

  1. Patient has had a recent event that caused a decline from prior function.
  2. Rehab Medicine must be a consulted service.
  3. Must be able to participate in 2 hours of consecutive therapy for 2 to 3 days per week.
  4. Demonstrating emerging skills and progress toward functional goals.
  5. Assessed by a COA PT & OT

Current time slots:

  • Team A: Monday, Tuesday, Thursday (1pm-3pm)
  • Team B: Tuesday, Thursday, Friday (10am-12pm) 
    *Which time slot a family is offered will depend on current/ earliest availability in the program

Referral Process

Referrals will need to come from your child’s provider and sent to: TransRehab@childrensal.org 

  • Typically, this provider is from the Rehab Medicine team.
  • If patient meets criteria for Trans Rehab program, please indicate “Transitional Rehab.”

The goal is to see these patients within 2 weeks of discharge if appropriate for the program

  • Allows them to get into a routine out of the hospital.
  • Caregivers and patients can identify any barriers in the home.
  • Allows time for functional recovery.

A patient might not be ready for Transitional Rehab upon discharge but may become appropriate within that first 18 months post injury. At this time COA PT & OT will assess and make considerations for scheduling in the Transitional Rehab Program to allow for a burst of therapy for up to 4 weeks.

Participation Requirements:

  • Family or Caregiver participation in sessions
  • Two no show or no calls without a medical excuse will result in removal from the program.
  • A patient who incurs a medical life event leading them to miss more that 2 sessions may be asked to postpone program until full attendance can be achieved.
  • Insurance approval or signed agreement to pay out of pocket.
