Children's at Lakeshore
Children’s of Alabama and Lakeshore Foundation have partnered to provide clinical and therapy services on the campus of Lakeshore Foundation. Patients are only accepted to Children’s at Lakeshore through the referral of a Children’s of Alabama physician based on medical needs and readiness for therapies.
Children’s at Lakeshore offers physician care provided by the UAB Division of Pediatric Rehabilitation Medicine and therapy services provided by Children’s of Alabama Physical, Occupational, and Speech Therapy staff members, with the added benefit of the focus on sports, fitness, recreation, and healthy living available at Lakeshore Foundation.
Children’s at Lakeshore serves children and adolescents with acute onset or chronically disabling conditions who can benefit from the combination of a medically-based model of care and community- based healthy living and transitional services. This unique collaboration is designed to help each child achieve their highest level of independence.
Children’s at Lakeshore Facilities include:
- Three medical exam rooms for the Pediatric Rehabilitation Medicine Clinic
- PT and OT gym and exercise area
- Two Speech Pathology treatment rooms
- Conference room
- Street level parking with an ADA compliant entry
- ADA compliant facilities and outdoor areas

Thank you for choosing Children’s South Outpatient Specialty Clinics to care for your child. We are a freestanding outpatient center located off I-459 on Acton Road. Free surface lot patient parking/handicapped parking is available in the front of the facility.

Preparing for Your Appointment
- Doctor’s name(s) and clinic location
- List of child’s medication(s)
- List of questions you may have for the doctor or nurse
- Insurance card
- Co-payment for insurance if indicated
- Insurance and demographic (address, phone #, etc.) information are confirmed at every visit
- Books or games
- Snacks or formula (please do not bring food if your child must not have anything to eat or drink before the appointment).
Permission to Treat
At least one parent or legal guardian must accompany all minor patients. If someone other than a legal guardian or parent will accompany your child, please ensure you provide them with a written, signed “Permission to Treat” slip. Please include the name of the person who will accompany your child as well as their relationship to your child in the permission slip.
Insurance/Payment/Referral/Billing
Please make sure you have a referral from your Primary Care Physician before the specialty visit, if this is required by your insurance. Know the services your insurance plan covers so that you can prepare for and be involved in the care of your child at Children’s of Alabama.
You may receive more than one bill for the care provided to your child at Children’s of Alabama. The bill you receive from Children’s of Alabama will not include the doctor’s fee or reading fee for any x-ray or lab work. If you have questions about your Children’s bill, please call: 205-638-9139.
