Preparing for Your Visit (Urology)

Scheduling:
To make an appointment with one of our pediatric urologists, the following should be faxed from the referring physician’s office:

Office/clinic phone number: 205-638-9840
Office fax number: 205-638-2515

Important Tips:

  • Your first visit with the urologist is a consultation visit. No surgery will be performed at this visit.
  • Be sure to bring your parking ticket to clinic to be validated for free parking.
  • Please arrive 30 minutes early to allow for parking and registration. Late arrivals may be asked to reschedule or may be seen at a later time.
  • Long Waits in Clinic: Your office visit *might* take 2 hours or longer, so come prepared. There are several common reasons for our extended wait times: Our doctors are sometimes called from clinic to perform emergency surgery; Patients with urgent needs must be given priority over scheduled patients; Many patients are required to have imaging and/or labs; Infants (less than 2 months of age) must be given priority because their ability to fight infection is not as strong as older children; We see a high volume of patients every day. We thank you in advance for your patience and understanding.
  • Insurance Cards, Insurance Referrals, and Coverage: Verify with your insurance company that the physician is covered under your plan. Be sure to bring your insurance card. It is your responsibility to verify with your insurance company or your referring physician that the appropriate referrals and/or pre-certifications have been completed prior to your appointment.
  • Permission to Treat: If a parent or legal guardian is not able to bring the child to the appointment, please provide a note including your child’s name, the name of the person who will bring your child, and their relationship to your child. State you are giving that person permission to obtain medical treatment for your child. Sign the note and include your phone number. The person bringing your child should know the child’s medical history.
  • If you need to reschedule or cancel your appointment, please call 205-638-9840. To avoid a possible no-show fee, please give at least 24-hour notice. This will allow us to offer that appointment time to another child needing urgent care. Due to high volume, we are currently scheduling new patient appointments approximately 4 months in advance.

What to bring:

  • A list of your child’s medications (name, dose, when taken)
  • Insurance card
  • Driver’s License
  • Name of your child’s primary doctor
  • Specialty physician co-payment for insurance, if applicable
