In order to provide the best care possible please fax the following information to 205-934-5544 or 205-638-9821 ***Due to the high volume of faxes we receive each day, Please call and ensure that your fax was received.
- Specialty Clinic Appointment Request Form with all information filled out
- Insurance referral if required or Updated EPSDT with Medicaid
- Any Lab results or x-ray reports
- Either a letter of referral from the physician or pertinent clinic notes including why patient referred and previous work up prior the visit.
Our Endocrinologists will review the information to determine if the patient needs to be worked in immediately. After the information is reviewed, you will receive a fax with the appointment date, time, and physician. Please allow 2-3 business days to receive the faxed appointment.
Cancellations:
Please make every effort to keep your scheduled appointment. If you are unable to do so, it may result in a delay in our ability to see your child. If you need to cancel an appointment, please call as soon as you know you are unable to keep the appointment so we can offer your child's appointment to another child.

What to Bring to an Appointment:
- Doctor's name
- Insurance card
- Co-payment for insurance, if applicable
- List of child's medications
- List of questions you may have for the doctor or nurse
- Books or games
- Snack or formula
- (Insurance and demographic information (address, phone #, etc.) will be confirmed at each visit)
- New Patient Information Sheets

The Children's Hospital of Birmingham Endocrine Clinic is committed to the quality care of your child. A vital part of our commitment is respect for your time. So in an effort to see everyone that came to their appointment on time in a timely manner.
Our late policy is as follows:
Effective 2/15/2010, we will be enforcing a late policy for all patients. If you are late for your appointment, you may be asked to reschedule or wait until the end of your doctor's clinic to be seen. This is to ensure that everyone is seen in a timely manner.
We ask that you arrive 15 minutes before your actual appointment to allow adequate time for registration. Arriving earlier than 150 minutes does not ensure that you will be seen before other reguarly scheduled patients that arrive after you for their appointment. We understand that finding our clinic downtown may be confusing the first time you visit us. Please allow ample time for traveling and possible traffic delays.

Wait Time:
I would like to take a few moments to tell you how important you and your child are to our clinic. Please allow me to explain a few of the common reasons we see patients and why there may be delays. A typical office visit may take a minimum of 2–2 ½ hours. The Endocrinology Clinic see's a large volume of patients for a broad spectrum of reasons. Here are a few of the most common reasons why:
- A child with New-Onset Diabetes or a regular Diabetes follow up may have the following done:
- a. Evaluation by M.D. or Nurse Practitioner
- b. Have intensive teaching by Diabetes Educator
- c. Have teaching by our Nutritionist
- d. Speak with our Social Worker
- e. Have labs drawn
- A child here for growth related issues may have the following done:
- a. Evaluation by M.D. or Nurse Practitioner
- b. If indicated, be scheduled for outpatient testing or MRI
- c. Have labs drawn and X-ray
- A child here with pituitary/adrenal/thyroid condition may have the following done:
- a. Evaluation by M.D. or Nurse Practitioner
- b. Intensive teaching by M.D., Nurse Practitioner, or Nurse Clinicianc. Stimulation tests (for specific adrenal patients) which require timed labs and medication administration
We are glad that you have chosen us to provide specialized care for your child. It is our goal to provide that care safely and efficiently. We understand that your time is valuable, and will do everything in our power to ensure that you are seen as close to your appointment time as possible. If you feel that your wait is too long, we will be happy to reschedule your appointment. Please take advantage of the report cards given out in the patient packets. Let us know how we're doing!
